There are some errors in Mr Wales' (The Govt. Inspector) report. For example, the list of names and ages (not shown here, but on page 137 of the book) is incorrect. It may be that these errors occurred when Mr Wales' original documents were transcribed into book form. In one instance on page 120 below, the date of the accident was given as the 26th instead of the 16th. I have underlined this error in red.
